kate died on november 2, 1940, 5 days before 90th birthday, of acute myocardial insufficiency, condition started showing symptoms of day before death. death certificate states suffered coronary artery disease , advanced arteriosclerosis. kate s death certificate contained significant discrepancies regarding parents names , birthplace. although born in hungary, death certificate states born in davenport, iowa, father marchal h. michael , mother catherine baldwin. birthplace of both parents shown on certificate unknown . superintendent of pioneer home named informant on death certificate.


kate buried on november 6, 1940, under name mary k. cummings in arizona pioneer home cemetery in prescott, arizona.
